All phlebotomists must be licensed in the state of Louisiana. The primary requirement for obtaining your license is to pass a nationally recognized examination. The following examinations are approved to be taken:
- American Medical Technologists
- American Society of Clinical Pathology
- American Society of Phlebotomy Technicians
- International Academy of Phlebotomy Services

After you pass the examination, the scores must be sent directly from one of the organizations above to the Louisiana State Board of Medical Examiners. Most of these examinations provided by national agencies require that you already have taken an approved training program and met certain requirements.
Training Programs and Classes
The state of Louisiana also requires that you have taken the proper phlebotomy training program approved by the state board. It is best to confirm with any program or school that they are approved to provide your phlebotomy class. You may also want to double check by calling or going online to the State of Louisiana’s website. After passing the approved program, you are typically able to then take the examination.
Students who have just finished an approved phlebotomy training program in Louisiana may apply for a temporary license until their final license is approved. To get a temporary license the applicant must have an examination date already setup. Once the exam is taken and passed, the licensee can become a permanent license. The cost of the temporary license is $40.
The application for the temporary license and permanent license is a couple of pages long. They both require that you pay a $40 to get a license. The application must also have a certificate of dean/registrar filled out and sent directly to the school where you completed your training. The program will then fill out the remainder of the form and send along a transcript. The applicant must also get a criminal background check completed. After submitting the application, you should receive a license that will allow you to perform the duties of a phlebotomist legally in the state of Louisiana.
